Job Description
Community Outreach & Referral Development Coordinator Quality Personal Care Atlanta, GA (Hybrid) Help Us Grow While Making a Difference Quality Personal Care is seeking an outgoing, relationship-driven professional to help expand our presence throughout the Atlanta community. If you enjoy networking, building professional relationships, and connecting people with services that improve their quality of life, we'd love to meet you. This role focuses on developing referral partnerships with healthcare providers and community organizations while representing one of Atlanta's growing home care agencies. What You'll Do Develop relationships with hospitals, physician offices, rehabilitation facilities, skilled nursing facilities, assisted living communities, social workers, case managers, discharge planners, VA organizations, and community partners. Conduct in-person outreach throughout the Atlanta metro area. Schedule and attend networking meetings and community events. Present Quality Personal Care's home care services to referral partners. Maintain relationships through consistent follow-up. Track outreach activities and referrals using our CRM. Work closely with leadership to identify new growth opportunities. Compensation This is an independent contractor (1099) opportunity with performance-based compensation . Client Referral Incentives 40+ authorized care hours per week $1,000 per new client $500 paid at start of care $500 paid after 30 days of active service 20-39 authorized care hours per week $600 per new client $300 at start $300 after 30 days Less than 20 authorized care hours per week $300 per new client $150 at start $150 after 30 days Monthly Performance Bonuses 5 new clients in one month: Additional $1,000 8 new clients in one month: Additional $2,000 Exceptional performers may qualify for additional incentive opportunities. There is no cap on earnings . Qualifications Strong communication and relationship-building skills Comfortable networking and meeting new people Self-motivated and able to work independently Reliable transportation Professional appearance and demeanor Healthcare, sales, marketing, outreach, or business development experience preferred Home care experience is a plus but not required Why Join Quality Personal Care?
