Job Description

Provider Relations & Community Outreach Coordinator - Part-Time A-One Home Healthcare Inc. is a Maryland DDA provider supporting adults with intellectual and developmental disabilities through person-centered services. We are seeking a motivated, relationship-focused Provider Relations & Community Outreach Coordinator to help expand A-One's visibility, develop community partnerships, and build meaningful relationships with professionals and organizations that serve individuals with developmental disabilities. This is a part-time, hybrid and field-based position . The successful candidate should be comfortable conducting in-person outreach as well as completing follow-up calls, emails, documentation, and relationship management remotely. Key Responsibilities Develop and maintain professional relationships with Coordinators of Community Services (CCSs), case managers, support planners, hospital social workers/discharge planners, transition professionals, community organizations, families, and other referral partners. Conduct regular in-person community outreach to introduce A-One and educate potential partners about our DDA services. Identify new referral and partnership opportunities throughout the communities A-One serves. Schedule and attend meetings, provider introductions, networking events, transition fairs, community events, and other outreach opportunities. Follow up consistently with professional contacts by phone, email, LinkedIn, and in person. Present A-One's services professionally to individuals, families, healthcare professionals, and community partners. Maintain an organized outreach/contact tracker documenting contacts, follow-ups, meetings, opportunities, and outcomes. Provide a brief weekly outreach report summarizing activities, contacts made, follow-up needs, and developing opportunities. Collaborate with A-One leadership on outreach priorities, community partnerships, and provider-relations strategies. Represent A-One professionally and in accordance with confidentiality, compliance, and ethical standards. What We Are Looking For Experience in community outreach, provider relations, case management, healthcare, social services, human services, developmental disability services, business development, or a related field. Familiarity with Maryland's developmental disability, healthcare, Medicaid, case-management, or community-service system is strongly preferred. Existing professional relationships within the Maryland DDA/IDD community are a plus. Strong interpersonal, communication, networking, and relationship-building skills. Comfortable making professional introductions and conducting both in-person and telephone outreach. Strong follow-up and organizational skills. Ability to work independently while maintaining clear communication with leadership. Working knowledge of email, Microsoft Office/Google Workspace, spreadsheets, and basic contact tracking. Reliable transportation and ability to travel locally for outreach meetings and events.
Schedule & Compensation Part-time:
approximately 20 hours per week $20.00-$24.00 per hour , based on relevant experience Hybrid/field-based position Flexible scheduling based on outreach needs Occasional meetings at A-One's Owings Mills office About A-One Home Healthcare Inc.
