Job Description Position Summary:

The Community Liaison & Outreach Coordinator is responsible for representing the home care agency within the community and developing strong relationships with healthcare professionals, senior organizations, community partners, families, and other referral sources. This position focuses on increasing community awareness of the agency's services, developing referral relationships, and ensuring the agency maintains a positive and professional presence throughout the community.

Essential Duties & Responsibilities:

Develop and maintain professional relationships with hospitals, clinics, physicians, social workers, discharge planners, senior centers, rehabilitation facilities, assisted living communities, and other potential referral sources. Conduct regular community outreach and visit referral partners to introduce and promote the agency's services. Provide information about the agency's home care services, eligibility, and referral process. Identify new community partnerships and referral opportunities. Attend health fairs, community events, senior expos, networking events, and other outreach activities. Coordinate and distribute agency marketing materials, brochures, business cards, and informational resources. Follow up with referral sources and prospective clients in a timely and professional manner. Maintain accurate records of community contacts, outreach activities, referrals, and follow-up efforts. Work closely with the administrative and intake teams to communicate new referrals and potential client needs. Help educate families and community partners about available home care services. Represent the agency professionally and maintain a positive image within the community. Assist with planning agency-sponsored community events, educational presentations, appreciation events, and outreach campaigns. Monitor community needs and identify opportunities for the agency to better serve local seniors, individuals with disabilities, and their families. Perform other duties related to community relations, outreach, and business development as assigned.
