This is an opportunity to make a meaningful difference in the lives of individuals experiencing substance use disorder (SUD) by helping them connect with treatment and take the next steps toward recovery. The focus of this position is community outreach and treatment engagement . You will meet people where they are and work closely with jails, courts, hospitals, treatment centers, healthcare providers, and other community partners to identify individuals who need help and connect them with appropriate substance use disorder treatment and recovery services. We are looking for someone who is comfortable working with people in challenging circumstances, can build trust quickly, and enjoys finding creative solutions to barriers that can make it difficult for someone to enter treatment. Role & Responsibilities Conduct community outreach to individuals experiencing substance use disorder and/or serious mental illness. Engage individuals who may be ready for or in need of substance use disorder treatment. Build trusting, respectful relationships with clients while meeting them where they are. Help individuals navigate the process of getting connected to appropriate treatment and recovery services. Work closely with local jails, courts, hospitals, emergency departments, treatment centers, and other community partners to identify treatment needs and facilitate timely connections to care. Coordinate referrals and communicate with treatment providers to support successful transitions into treatment. Follow up with individuals to encourage treatment engagement and support continued participation in recovery services. Conduct assessments to identify substance use, behavioral health, and treatment needs. Develop individualized care plans based on identified needs and treatment goals. Monitor progress and modify care plans as appropriate. Advocate for clients and help address barriers that may prevent them from accessing or engaging in treatment. Make community-based visits to client residences, jails, hospitals, treatment facilities, and other locations as needed. Maintain consistent communication with clients, treatment providers, justice-system partners, healthcare professionals, and community organizations. Establish and maintain strong working relationships with community resources and referral partners.
