About New Flower Homes New Flower Homes is a small, welcoming Community-Based Residential Facility (CBRF) located in Grafton, Wisconsin. We are committed to providing compassionate, individualized care in a comfortable, home-like environment. We are looking for an energetic and dependable Community Outreach & Resident Care Coordinator who can help us grow our resident community while also supporting resident care. This is a unique position combining marketing, community outreach, resident recruitment/admissions, and caregiving. The ideal candidate has experience in senior living, assisted living, CBRF, healthcare marketing, caregiving, or a related field and enjoys building relationships within the community. Primary Responsibilities Resident Recruitment, Marketing & Community Outreach A major responsibility of this position is helping New Flower Homes identify and connect with prospective residents and referral sources.
Responsibilities include:
Develop and maintain relationships with hospitals, rehabilitation facilities, skilled nursing facilities, clinics, social workers, case managers, discharge planners, ADRCs, senior organizations, and other community partners. Identify individuals and families who may benefit from placement at New Flower Homes. Follow up promptly with resident inquiries and referrals. Communicate with prospective residents and their families regarding New Flower Homes and the services we provide. Help coordinate facility tours and meetings with prospective residents and families. Maintain regular communication with referral sources. Conduct community outreach and help increase awareness of New Flower Homes. Assist with online and local marketing activities, including social media and community events. Maintain organized records of referrals, inquiries, follow-ups, tours, and potential admissions. Work with management to develop strategies to increase resident census and maintain strong community relationships. Represent New Flower Homes professionally and positively in the community. Resident Care & Caregiving When residents are admitted, this position will also participate in direct resident care as needed.
Responsibilities may include:
Assist residents with activities of daily living (ADLs), including dressing, grooming, toileting, bathing, mobility, and other personal care. Provide companionship and emotional support. Assist with meals and daily activities. Support residents in maintaining independence and dignity. Observe residents and report changes in condition to appropriate staff. Follow resident care plans and facility policies. Assist with medication administration when properly trained and authorized. Maintain a clean, safe, respectful, and comfortable environment. Complete required documentation. Work cooperatively with caregivers, management, healthcare professionals, residents, and families. Qualifications Preferred candidates will have BOTH marketing/outreach skills and caregiving experience. Required/Preferred Experience Previous experience in marketing, sales, community outreach, admissions, business development, or resident recruitment strongly preferred. Previous experience working in assisted living, CBRF, senior living, home care, healthcare, or long-term care preferred. Caregiving experience required. Wisconsin CBRF certification or CNA certification strongly preferred. Experience working with older adults, individuals with physical disabilities, or residents with dementia is a plus. Strong communication and interpersonal skills. Comfortable speaking with families, social workers, case managers, healthcare professionals, and community organizations. Ability to build professional relationships and follow up consistently. Self-motivated and comfortable working independently. Organized and able to track multiple referrals and prospective residents. Basic computer, email, and social media skills. Dependable transportation and ability to travel locally for outreach activities. Must meet all applicable Wisconsin CBRF employee requirements.
