Clinical Liaison (Senior Living) Village at Cook Springs
Pell City, AL Pay Rate Frequency:
Hourly Summary of Duties Responsible for coordinating skilled nursing admissions using independent judgment and discretion. Directs admission process between physicians, other referral sources, and the facility staff to efficiently evaluate and admit residents. Develop and implements marketing strategies for referral source development and census growth. Essential Job Functions 1. Screens referrals for admissions and discusses potential admissions with the staff. 2. Directs and processes admissions and serves as the liaison between the facility staff, hospitals, admitting/attending physicians, and other referral sources. 3. Develops a system for coordinating, tracking, and monitoring referrals and serves as initial point-of-contact for all referrals/inquires. Conducts facility tours as needed. 4. Interviews residents and family members to obtain psychosocial information and complete comprehensive assessments of the psychosocial needs of each resident. 5. Conducts and documents assessments as necessary. 6. Completes and submits admission assessments and statistical reports as requested. 7. Processes referrals and inquires. Maintains updated communication in referral source platforms. 8. Maintains an accurate record of available beds/units. 9. Communicates the care needs and medical equipment needs of residents upon admission with the facility staff and assist as indicated. 10. Conducts rounds and provides information to appropriate staff to ensure readiness for admissions. 11. Obtains necessary medical records for admissions, i.e., diagnosis, physician orders, OBRA screening, etc. 12. Plans and coordinates special events and community engagement initiatives that promote facility services to potential residents and families. 13. Assumes additional responsibilities and special projects as assigned or needed. Education & Experience Must possess a degree in Nursing or business related field. Bachelor Degree preferred. Minimum of two (2) years clinical or healthcare admissions/marketing experience required.
