Clinical Liaison Skilled Nursing & Rehabilitation | Full-Time A respected skilled nursing and rehabilitation facility is seeking an experienced Clinical Liaison to join its team. This position is ideal for a healthcare professional who is skilled at developing referral relationships, communicating with healthcare partners, and helping patients and families navigate the transition from hospital to post-acute care. The Role The Clinical Liaison will serve as a key connection between the facility and the healthcare community. This individual will spend time developing relationships with hospitals and referral partners while supporting the referral and admissions process. What You'll Be Responsible For Build and maintain relationships with hospital case managers, social workers, physicians, discharge planners, and other referral sources. Conduct regular hospital visits and maintain consistent communication with referral partners. Identify qualified skilled nursing and rehabilitation referral opportunities. Respond promptly to referrals and collaborate with admissions and clinical teams. Meet with patients and families to discuss post-acute care options when appropriate. Assist with clinical evaluations and facility selection. Follow referrals from initial inquiry through admission or final disposition. Promote the facility's clinical services, rehabilitation programs, and capabilities. Identify new referral opportunities and expand relationships within the healthcare community. Participate in provider meetings, educational presentations, networking events, and community outreach. Maintain accurate records of referrals, follow-ups, hospital visits, and other activities. Communicate referral trends, barriers, and opportunities to leadership. What You Bring Previous experience in clinical liaison, hospital liaison, skilled nursing admissions, or healthcare marketing within a skilled nursing or long-term care setting preferred. Experience working with hospitals, case managers, social workers, discharge planners, and physicians. Knowledge of skilled nursing, rehabilitation, long-term care, or post-acute services. Strong communication, presentation, and relationship-building skills. Excellent organizational and follow-up abilities. Professional and confident when interacting with healthcare professionals and families. Ability to work independently while collaborating with clinical and admissions teams. Comfortable working in hospitals and throughout the community. Proficiency with Microsoft Office and CRM/EHR systems. Valid driver's license and reliable transportation required. Healthcare credential such as RN, LPN/LVN, LSW, or related clinical credential preferred , depending on the scope of the position. Why Consider This Opportunity?
