Job Description

Admissions (Clinical) Liaison - Roanoke and Martinsville-Danville Markets A multi-site skilled nursing provider is seeking an experienced, energetic Admissions (Clinical) Liaison to represent four skilled nursing facilities across the Roanoke and Martinsville-Danville markets. This highly visible, relationship-driven role will spend significant time onsite at area hospitals, conducting bedside visits and clinical assessments while building strong partnerships with case managers, social workers, physicians, and other referral sources. Key Responsibilities Partner with the Central Intake Department to coordinate and evaluate patient referrals Conduct timely bedside visits and onsite clinical assessments Build and maintain strong relationships with hospital case managers, social workers, physicians, and other referral partners Identify opportunities to increase qualified referrals and admissions Manage the assigned territory efficiently and cost-effectively Monitor market trends and maintain a current competitive analysis Follow all company policies and procedures Qualifications Bachelor's degree preferred; relevant experience may be considered in place of formal education Current Licensed Practical Nurse (LPN) credential preferred, but not required At least two years of skilled nursing, post-acute care, hospital liaison, or healthcare marketing experience preferred Strong knowledge of skilled nursing, rehabilitation, and long-term care services Demonstrated success in referral development and census growth Excellent communication, relationship-building, and organizational skills Valid driver's license, clean driving record, and ability to travel throughout the assigned markets
Compensation and Benefits Job Type:
Full-time Salary:
