POSITION SUMMARY
Using holistic approach in providing medical social services to elders in Utuqqanaat Inaat (long term care), and supporting swing bed admissions. The role focuses on proactive, integrated, supportive care to allow residents and patients to live as independently as possible.
PRINCIPAL DUTIES AND RESPONSIBILITIES
Develop social histories of patients and residents and develops a social assessment and contributes to development of care plans. Participates in discharge planning. Participates in interdisciplinary care plan conferences and contributes in the development of care plans for patients. Support advance care planning (Advance Directives, POLST, etc.). Assist patients and residents in financial, housing, legal and long-term health needs. Complete swing bed admission assessments and support subacute rehab care. Maintain accurate and timely documentation in compliance with state/federal regulations and policy. Prioritize and manage multiple tasks in a dynamic environment. Completes the Social Services section of the MDS. May coordinate admissions process for new or returning residents. Coordinates social services in-service and educational programs for staff.
Job Qualifications:
MINIMUM REQUIREMENTS
Licensed Bachelor of Social Work I Bachelor of Social Work (BSW) from an accredited school of Social Work. State of Alaska license at BSW (LBSW) required. One-year experience in hospital or health-care setting. Basic knowledge of computer use. Prior experience working in cross cultural setting with Native Alaskan populations. Licensed Master of Social Work II Same as LBSW, but with Masters of Social Work (BSW) from an accredited school of Social Work and State of Alaska License at MSW (LMSW) required.
