$3,300 - $3,400 / Weekly Job Overview We are seeking a dedicated and compassionate Licensed Social Worker to join our healthcare team. The ideal candidate will possess a strong commitment to patient-centered care and demonstrate expertise in clinical social work practices across diverse medical settings. As a Licensed Social Worker, you will provide vital psychosocial support, coordinate community resources, and assist patients and their families through complex healthcare journeys. Your role will be instrumental in promoting emotional well-being, facilitating discharge planning, and ensuring compliance with healthcare regulations. Responsibilities Conduct comprehensive patient assessments to identify psychosocial needs, including mental health status, family dynamics, and social determinants of health. Provide individual, family, and group counseling utilizing evidence-based approaches such as behavioral therapy, motivational interviewing, and clinical counseling techniques. Coordinate with interdisciplinary teams to develop and implement personalized care plans that address social and emotional factors impacting health outcomes. Facilitate access to community resources, including housing, financial assistance, mental health services, and disability support programs. Assist with discharge planning by evaluating patient needs post-hospitalization or post-acute care, ensuring smooth transitions to home or assisted living environments. Maintain accurate medical documentation in electronic medical records (EMR) systems while adhering to HIPAA guidelines and social work practice standards. Engage in crisis intervention when necessary, providing immediate support for patients experiencing acute emotional distress or behavioral health crises. Qualifications Valid licensure as a Licensed Clinical Social Worker (LCSW) or equivalent credential from an accredited licensing board. Proven experience working in hospital settings, hospice care, geriatrics, pediatrics, or community health environments. Strong knowledge of medical terminology, clinical ethical guidelines, and healthcare regulations including HIPAA and Medicare policies. Familiarity with behavioral health interventions such as cognitive behavioral therapy (CBT), applied behavior analysis (ABA), and trauma-informed care strategies. Experience working with diverse populations, including individuals with developmental disabilities, autism spectrum disorder (ASD), dementia or Alzheimer's disease, PTSD, and end-of-life care scenarios. Proficiency in case management practices, discharge planning processes, and use of electronic medical records (EMR) systems. Excellent communication skills with the ability to collaborate effectively within interprofessional teams and engage patients empathetically.
