The Service Coordinator is responsible for linking residents with supportive services that promote independence, improve quality of life, and help residents remain safely housed. This position conducts resident assessments, develops service plans, coordinates educational and wellness programs, provides referrals to community resources, maintains required documentation, and collaborates with property management and local service providers while ensuring compliance with HUD regulations and company policies.
What You'll Do:
Empower residents to live independently by connecting them with essential services, community resources, healthcare providers, and social service agencies. Build and maintain a robust network of local agencies, providers, and wellness resources. Educate and guide residents and families through Federal, State, and local programs tailored to senior adults. Assist residents in accessing benefits such as Medicare, Medicaid, SNAP, SSI, transportation, and other assistance programs. Coordinate wellness and educational programs that promote healthy aging and independent living. Conduct regular follow-up with residents to ensure services remain appropriate and effective. Maintain accurate resident files and documentation in compliance with HUD and company requirements. Complete assessments, referrals, case notes, and reports in a timely manner. Respond to resident concerns and assist with problem-solving. Build relationships with local service providers to expand available resources for residents. Collaborate with the management team to identify residents in need and provide proactive assistance. Advocate and negotiate on behalf of residents for cost-effective and adequate services. Create and maintain an up-to-date directory of community resources for residents and staff.
What You'll Bring:
Bachelor's degree in Social Work, Gerontology, Psychology, or related field ( Required ) Knowledge of local services, state and federal programs, and senior resources. Understanding of Medicare, Medicaid, Social Security and other public benefit programs for seniors. Strong case management and problem-solving skills with a team-oriented attitude. Excellent communication and relationship-building skills Organized, resourceful, and ready to take initiative
