Key Responsibilities Conduct patient assessments to identify social service needs. Develop and implement personalized care plans in collaboration with an interdisciplinary team. Provide emotional support and counseling to patients and families. Advocate for patient needs within the facility and coordinate with external resources. Maintain accurate and confidential patient records. Oversee discharge planning and ensure continuity of care post-discharge. Qualifications Social Work License valid in the state of Alabama Minimum 1 year of experience in Social Work or a related field. Experience in a long-term care or healthcare setting, particularly in social services. Strong understanding of case management and care coordination. Exceptional communication and interpersonal skills. Ability to work compassionately with a diverse patient population.
