Responsibilities:

Provide social work services to patients and their families Develop and implement individualized treatment plans to address patients' social, emotional, and behavioral needs Collaborate with interdisciplinary teams to ensure comprehensive care and address patients' concerns Conduct assessments and screenings to identify patients' social and emotional needs Provide crisis intervention and behavioral services as needed Facilitate patient education and support groups Maintain accurate records and documentation of patient services About the
