$60,000 - $75,000 per year, depending on experience Make a Difference When It Matters Most Maggie's Hospice is seeking a compassionate and dedicated Social Worker to join our team serving patients and families throughout the Verde Valley. As a Hospice Social Worker, you will provide emotional support, counseling, advocacy, and resource coordination to individuals facing serious illness and end-of-life transitions. You will work alongside an experienced interdisciplinary team committed to delivering exceptional care with dignity, compassion, and respect. Responsibilities Conduct psychosocial assessments of patients and families to identify emotional, social, financial, and support needs. Develop and implement individualized plans of care in collaboration with the interdisciplinary team. Provide counseling, emotional support, and grief-related interventions to patients, families, and caregivers. Assist with advance care planning and end-of-life decision-making. Connect patients and families with community resources and support services. Advocate for patient goals, preferences, and quality of life. Participate in interdisciplinary team meetings and contribute to comprehensive care planning. Maintain timely, accurate, and compliant documentation within the electronic medical record. Support patients and families through transitions in care and bereavement-related needs. Qualifications Bachelor's Degree in Social Work (BSW) required. Master's Degree in Social Work (MSW) preferred. Hospice, palliative care, home health, hospital, skilled nursing, behavioral health, or related healthcare experience preferred. Strong communication, counseling, and interpersonal skills. Ability to work independently while maintaining strong collaboration with interdisciplinary team members. Knowledge of community resources and patient advocacy principles. Current driver's license, reliable transportation, and ability to travel within the assigned territory.
