Job Description

Stillwater Hospice is seeking an experienced Hospice Medical Social Worker to join our compassionate interdisciplinary team serving patients and families in the Peoria/Glendale/Phoenix Area of Phoenix. This role is ideal for someone who is deeply committed to end-of-life care, values connection and dignity, and understands the unique emotional, psychosocial, and practical needs of hospice patients and their loved ones. Requirements (must have): Current MSW license in Arizona Previous hospice experience (required) Strong understanding of hospice philosophy, goals of care, and interdisciplinary teamwork Excellent communication, documentation, and organizational skills Ability to work independently in the field while collaborating closely with the care team Responsibilities include Providing psychosocial assessments and ongoing emotional support to hospice patients and families Assisting with advance care planning, resources, and end-of-life education Supporting caregivers through anticipatory grief and bereavement needs Collaborating with nurses, chaplains, aides, and physicians to ensure holistic care Maintaining timely and accurate documentation in compliance with hospice regulations
