New Grads Welcome! The Social Worker will work with patients to promote positive mental health and emotional well-being. The ideal candidate will have a passion for helping others and a drive to make a difference in someone's life.
Responsibilities:
Completes Psychosocial assessments on patients on assigned unit according to the guidelines set forth in the Procedures for Psychosocial Assessment. Participates in Treatment planning for assigned patients. Regularly consults and complies with all mandated reporting laws concerning child abuse/neglect, elder/dependent adult abuse/neglect, domestic violence, and Tarasoff notifications in a timely manner. Regularly acts as a liaison between the hospital and other agencies and the court system when a patient on their caseload is so involved. Arranges with outside agencies to coordinate a discharge and aftercare plan to meet assessed needs. Anticipates patient needs and provides advisement to multidisciplinary team regarding meeting those needs. Demonstrates knowledge of assigned cases; to include, but not limited to the current clinical status, discharge plans, and legal status. Properly demonstrates behavior modification techniques. Provides individual, group and family therapy for patients when needed. Submits completed temporary conservatorship applications in a timely manner. Effectively performs additional duties and responsibilities as position requires.
Education & Experience:
Master's Degree required. May be a ASW, APCC, LPCC, LCSW, MFT or registered AMFT. Social Work training and experience preferred. Fluent in Spanish preferred.
