(Exodus) is looking to hire individuals to initiate contacts with a variety of community resources; and implement linkages and discharge planning for clients in the Exodus Recovery Urgent Care Center. The Discharge Planner shall meet the qualifications as indicated in the Short-Doyle Medi-Cal Rehab Option Manual and provide services in accordance with the scope of their practice. Schedule for this position will be five 8-hour days, Sunday- Thursday.
MINIMUM QUALIFICATIONS
Bachelor's Degree required. Experience with case management with exposure to populations of Chemical dependency and mental health and working within interdisciplinary team and outside community agencies is highly desired. Strong verbal and written communication skills. Strong knowledge of community resources. Ability to work independently and shows mature judgment.
EDUCATION
Bachelor's degree required. Master's degree preferred in a mental health related field; RN, PhD, Psy
D, LCSW, MFT
preferred. Licensed eligible desirable. Current CPR Certification and current CPI Certification preferred.
