Our center offers nursing care, therapy services, and engaging therapeutic activities designed to enhance the quality of life for our participants. Summary We are seeking a compassionate and skilled Social Worker to join our team at Magnolia Adult Day Health Care. In this role, you will play a vital part in supporting our clients' emotional and social well-being while collaborating with healthcare professionals to provide holistic care. Responsibilities Conduct assessments to determine clients' needs and develop individualized care plans. Provide clinical counseling and support to clients and their families. Facilitate therapeutic activities that promote social interaction and emotional well-being. Ensure compliance with HIPAA regulations and maintain client confidentiality. Collaborate with multidisciplinary teams to coordinate care and services for clients. Collect data for reporting purposes and evaluate the effectiveness of interventions. Requirements Master's degree in Social Work, Psychology, Counseling or Gerontology from an accredited program. OR Bachelors degree in Social Work PLUS 2 years of experience in community based adult program (CBAS).
