In this vital role, you will provide emotional support, counseling, and resource coordination to patients facing end-of-life challenges and their families. Your expertise will help ensure that patients receive comprehensive, person-centered care that respects their dignity and cultural backgrounds. This paid position offers an opportunity to make a meaningful difference in the lives of individuals during their most vulnerable moments while working within a supportive, collaborative environment. Duties Conduct thorough patient assessments to determine social, emotional, and practical needs related to hospice and palliative care. Provide individual, family, and group counseling utilizing clinical techniques such as motivational interviewing, cognitive behavioral therapy, and crisis intervention. Coordinate community resources, including home care services, support groups, legal assistance, and financial aid programs like Medicare. Assist with discharge planning by evaluating patient needs and collaborating with medical teams to develop personalized care plans. Maintain accurate medical documentation in electronic medical records (EMR) systems in compliance with HIPAA regulations and social work practice standards. Support patients with complex emotional issues such as grief, trauma (including PTSD), dementia-related conditions like Alzheimer's disease, or developmental disabilities. Participate in interdisciplinary team meetings to ensure holistic patient care and effective communication among healthcare providers. Skills Extensive experience in hospice care, end-of-life care, or palliative medicine with a focus on compassionate patient support. Proficiency in social work practice standards, medical terminology, ICD coding, and clinical ethical guidelines. Strong skills in client communication, diagnostic evaluation, case management, and crisis management across diverse populations. Knowledge of community resource coordination tailored for geriatric patients, individuals with disabilities, or those requiring Alzheimer's or dementia care. Ability to perform behavioral health interventions including behavioral therapy, mental health counseling, and applied behavior analysis when applicable. Familiarity with EMR systems and medical records management ensuring compliance with HIPAA regulations. Experience working with individuals from diverse cultural backgrounds and those with complex medical conditions such as cancer or chronic illnesses. Certification in basic life support (BLS) and CPR; experience in inpatient settings or nursing homes is highly valued.
