Job Summary:

The Wraparound Facilitator serves as the first point of contact and service provider for adoptive families in a newly developing service area. This unique position leads the High Fidelity Wraparound (HFW) process for enrolled families and holds responsibility for outreach efforts to expand program awareness and client engagement in the region. As the first staff member in the area, this Facilitator will provide comprehensive wraparound facilitation, including assessment, planning, coordination, and team facilitation. They will also play a key role in building partnerships, increasing referrals, and helping establish a sustainable Wraparound presence in the community. Once three families are enrolled, additional staff will be hired to support the area's continued growth. The ideal candidate will be bilingual in English and Spanish and have experience in social work, family engagement, or community-based mental health services.

Wraparound Facilitation Duties:

Provide full Wraparound facilitation services for enrolled families using the four phases of the High Fidelity Wraparound model: Engagement, Planning, Implementation, and Transition. Develop, monitor, and revise individualized Plans of Care in collaboration with the Child and Family Team (CFT). Facilitate CFT meetings, ensuring family voice and choice, team consensus, and coordinated implementation of supports. Conduct strengths and needs assessments, including the use of tools such as CANS and WFI-EZ. Coordinate formal and informal supports to ensure that the family's needs and goals are addressed holistically. Support crisis planning, safety planning, and problem-solving as needed. Maintain accurate documentation for billing, compliance, and progress tracking. Maintain fidelity to the HFW model, using tools, supervision, and team support to ensure adherence.

Community Outreach and Area Development:

Serve as the local representative of the Wraparound program to build trust and awareness within the community. Conduct outreach to schools, behavioral health providers, community organizations, and referral sources to increase program visibility and family enrollment. Assist new families through the onboarding and intake process, supporting their transition into Wraparound services. Track and report outreach activity and assist in identifying trends, needs, and barriers in the region. Collaborate with leadership on developing the service area and planning for future staff and resource needs.

Required Qualifications:

Bilingual in Spanish and English. Bachelor's degree in Social Work, Psychology, Human Services, or related field. Demonstrated experience working with families in community-based, trauma-informed, or wraparound settings. Strong facilitation, planning, and engagement skills.

Preferred Qualifications:

Experience with CANS assessments. Understanding of the High Fidelity Wraparound model.

Additional Requirements:

Valid California driver's license, auto insurance, and reliable transportation. Cleared background check and current compliance with required trainings. Flexibility to travel and adjust work hours to accommodate family needs and outreach opportunities.

Physical Demands:

Ability to sit, drive, and use a computer for extended periods. Frequent travel within the assigned region. Occasional physical activity during family visits or community events. Reasonable accommodations provided as required by law.

$72,000.00 - $80,000.00 per year
