A Family, School, or General Social Worker provides specialized assistance to vulnerable families and children. Intervenes in crisis situations; evaluates individual and family needs, helps clients get access to health care, mental health care or social services such as housing assistance. Advocates for clients and provides follow up visits. May work with teachers, parents and school administrators to help students improve academically and deal with social issues such as bullying.

Job Description

Bright Horizon Adult Day Health Care is seeking one (1) Medical Social Worker for social work services for participants. Duties and Responsibilities:
Provide all social work services for participants. Meets with family members or referring entities and determines appropriate program. Completes social service assessments on all participants. Ensures person centered care planning, includes participant, family members and/or caregivers in the development of care plan. Provides discharge planning for all participants. Provides participant and family counseling as needed.
Qualifications:
Master's degree in social work from an accredited school of social work or a master's degree in psychology, gerontology, or counseling from an accredited school One (1) year of experience providing social services in one or more of the fields of aging, health, or long-term care services or be licensed by the California Board of Behavioral Sciences. Bilingual is highly preferred. Compensation is based on experience.
Hours of Operation:
Monday through Friday, 8:00 am-4:30 pm.
