Job Summary We are seeking a compassionate and highly skilled Social Worker with a Master of Social Work (MSW) degree to join our dynamic healthcare team. The ideal candidate will possess extensive experience in medical social work, including patient assessment, discharge planning, and community resource coordination. This role offers an opportunity to make a meaningful impact by supporting patients through complex medical and psychosocial challenges within a multidisciplinary environment. Responsibilities Conduct thorough psychosocial assessments of patients to identify needs related to mental health, addiction, end-of-life care, and social support systems. Develop individualized care plans that incorporate behavioral therapy, clinical counseling, and community resource linkage. Collaborate closely with healthcare providers to facilitate patient discharge planning and coordinate post-acute care services. Provide crisis intervention and mental health counseling to patients experiencing emotional or psychological distress. Maintain accurate medical documentation in compliance with HIPAA regulations and clinical ethical guidelines. Assist patients from diverse cultural backgrounds by connecting them with appropriate community resources and support networks. Participate in interdisciplinary team meetings to ensure holistic patient care and effective clinical interprofessional collaboration. Requirements Master of Social Work (MSW) degree from an accredited institution; licensure as required by state regulations. Proven experience working in hospital settings, hospice care, or outpatient clinics with a focus on medical social work. Knowledge of medical terminology, ICD coding, Medicare policies, and electronic medical record (EMR) systems. Strong skills in client communication, case management, and behavioral health interventions such as motivational interviewing and cognitive behavioral therapy. Familiarity with hospice & palliative medicine, dementia care, Alzheimer's care, PTSD care, and working with geriatric patients preferred. Ability to handle crisis situations effectively while demonstrating cultural sensitivity and inclusivity. Certifications in basic life support (BLS) or CPR are highly desirable; experience working with individuals with disabilities or developmental disorders is a plus.
