ADHC/CBAS
is seeking a compassionate and organized Social Worker to join our multidisciplinary team and support adult and senior participants with medical, emotional, and social needs.Responsibilities:
Complete psychosocial assessments and care plans Participate in MDT meetings and reassessments Provide counseling, crisis support, and community resources Coordinate services with families, providers, health plans, and community agencies Assist with Enhanced Care Management (ECM) assessments, care planning, referrals, follow-up, and documentation Maintain accurate and timely case documentationRequirements:
MSW required Armenian speaking required Farsi and/or Russian preferred ADHC/CBAS, ECM, healthcare, geriatrics, or case management experience preferred Strong communication, documentation, and teamwork skills BBS supervision hours provided.Call/text Anna:
