Job Summary We are seeking a compassionate and dedicated Social Worker to join our dynamic Hospice, healthcare and community services team. Duties Conduct thorough patient assessments to identify needs related to mental health, developmental disabilities, addiction, or trauma such as PTSD. Provide individual, group, and family counseling utilizing evidence-based practices including cognitive behavioral therapy (CBT), psychodynamic therapy, and applied behavior analysis (ABA). Develop tailored care plans and coordinate services across medical records systems while ensuring compliance with HIPAA regulations. Facilitate crisis intervention and crisis management strategies to support patients experiencing acute mental health or behavioral issues. Engage in case management activities such as intake procedures, service planning, progress documentation, and follow-up care coordination. Utilize electronic medical records (EMR) systems for accurate data collection, documentation of medical terminology, and clinical research purposes. Collaborate with multidisciplinary teams including healthcare providers, educators in special education settings, and community organizations to optimize patient care outcomes. Stay informed on current best practices in psychopharmacology, ICD coding (ICD-9/10), CPT coding for billing purposes, and diagnostic evaluations for various conditions including autism spectrum disorders and Alzheimer's care. Skills Proven experience in psychotherapy modalities such as cognitive behavioral therapy (CBT), psychodynamic therapy, or behavioral therapy. Strong working knowledge of motivational interviewing techniques to foster patient engagement. Expertise in child & family counseling along with working with children with developmental disabilities or autism spectrum disorders. Familiarity with home & community care environments and delivering services within Geriatrics or pediatric settings. Experience with EMR systems for documentation, data collection, research activities, and medical records management. Knowledge of

coding systems for accurate diagnosis documentation; familiarity with CPT coding for billing processes. Ability to perform comprehensive patient assessments including intake interviews and diagnostic evaluations. Understanding of HIPAA regulations to maintain confidentiality and ensure secure handling of sensitive medical information.
