Job Description

We are seeking a passionate Social Work Case Manager to work in our Case Management department. The role would include case managing patients in the Sacramento, and Yolo counties. Social Work Case Manager Responsibilities:

Work as a team with an RN to provide supportive client centered

HIV/AIDS

case management services (AIDS Medi-Cal Waiver Program) to a diverse group of clients in their home setting. Complete biannual assessments and follow up monthly with clients in between home visits. Help clients remain connected to appropriate medical/mental health/AOD/community services with a goal of stabilizing their health, meeting their needs, and avoiding higher levels of care. Experience providing similar or related services and/or working with this population preferred.

Social Work Case Manager Education/Training:

Licensed Clinical Social Worker (LCSW), Marriage and Family Therapist, or Psychologist licensed by the State of California; OR a minimum of a bachelor's degree in Social Work, Psychology, Counseling, Rehabilitation, or Sociology.

Social Work Case Manager Experience:

One year of experience in clinical case management (required for candidates qualifying under the bachelor's degree pathway).
