Hospice Medical Social Worker (Per Diem)

Pay:

$20.00-$26.00+ mileage & sick time

Temecula, Murrieta, Hemet, Riverside, Moreno Valley, Redlands, San Bernardino, High Desert.

Schedule:

Mostly weekends, PRN, as needed. We're hiring two experienced Per Diem Medical Social Workers (MSWs) to join our growing hospice care team. If you're a compassionate clinician who thrives in providing emotional support and advocacy for patients and families, we'd love to meet you.

What You'll Do:

Conduct psychosocial assessments and provide counseling to patients and families Collaborate with the interdisciplinary team to create and support individualized plans of care Connect families with community resources and provide end-of-life support

Requirements:

Bachelor's Degree in Social Work (BSW) - required Minimum 1 year of healthcare experience within the last 3 years Valid driver's license, reliable insured vehicle, and current CPR certification Willingness to commute thought out the
