Our client is seeking a dedicated and compassionate Social Worker to join their team, playing a key role in providing high-quality, comprehensive care to frail seniors. The Social Worker will work closely with a diverse team of healthcare professionals to ensure that patients receive the best possible support and services. This role is ideal for individuals who are passionate about making a positive impact in the lives of seniors and fostering a holistic approach to their care.Setting :
Outpatient Clinic working with the geriatric population.Responsibilities :
Conduct social work assessments to identify the psychosocial needs, preferences, and goals of patients, contributing to the development of personalized care plans. Actively participate in team meetings to collaborate on care plan creation and patient management. Provide social work interventions as outlined in patient care plans, including resource arrangement, care transitions, individual and group counseling, and case management. Partner with primary care physicians and other team members to ensure smooth transitions of care between settings, such as hospitals, skilled nursing facilities, and home care. Coordinate, facilitate, and lead care conference meetings to ensure effective communication and care coordination with patients, families, caregivers, and team members. Offer discharge planning and support when patients disenroll from the program. Other duties as assigned Master's Degree in Social Work (MSW). A minimum of one (1) year of experience working with a frail or elderly population. Experience coordinating and facilitating care conference meetings. Perfered experience working with individuals who have behavioral health and substance abuse issues.Benefits:
