1. interview referrals to determine participation for full ADHC assessment; 2. conduct assessments of psychosocial needs of participants; 3. develop individualized care plans for each participant; 4. document services performed; 5. make referrals to available community resources; 6. develop peer support and family caregiver for participants; 7. liaison between families and caregivers / community service providers; 8. participate in community education, networking and advocacy groups serving frail and disabled older adults; 9. initiate discharge plans; 10. ensure center activities conform to applicable policies, health and safety standards.

Job Details:

1. interview and screen all referrals to determine the eligibility of the prospective participant by a full ADHC assessment; 2. conduct assessment of the participant using a person-centered approach and develop an individualized plan of care (IPC) for each participant with interdisciplinary team IDT and family members; 3. maintain signed and dated documentation of all services performed on the day of the service and signed and dated quarterly progress note; 4. actively participate in MDT meetings, attend Care Coordination meetings, staff/team meetings, participant council meetings or other meetings as assigned; 5. develop peer support groups and family caregiver groups; 6. serve as liaison to the participant's family and caregivers; 7. participate in community education, networking and advocacy for participants; 8. initiate the development of a discharge plan at admission and update the discharge plan at each reassessment period; assume responsibility for implementation of the plan at the time of discharge from the center; 9. conform to all applicable policies of the center and attend the orientation program offered by the center; 10. comply with all health and safety standards including licensing, fire and disaster preparedness regulation and agency policies.
