POSITION DUTIES

•Provides psychosocial assessments, emotional support, counseling, care coordination, resource referrals, and end-of-life guidance to patients and their families to help them navigate the physical, emotional, financial, and social challenges associated with terminal illness.

MINIMUM REQUIRED QUALIFICATIONS

•CO LCSW 2 years recent social work experience in a healthcare setting
