Job Summary:
As an LCSW in our Early Intervention Program you will provide critical support to children with developmental disabilities and their families, helping them navigate the challenges of early development. The LCSW will work closely with a team of professionals to assess, plan, and implement individualized service plans (ISPs) that promote the social, emotional, and behavioral well-being of the children we serve.
Responsibilities:
Conduct psychosocial assessments of children and families to identify needs, strengths, and developmental areas. Collaborate with interdisciplinary team members to understand each child's needs and contribute to care planning. Develop, implement, and update individualized service plans (ISPs) addressing social, emotional, and behavioral goals. Provide individual and family counseling, including crisis intervention and support for acute stressors. Serve as a primary contact for families, coordinating internal and external services and resources. Assist families in navigating early intervention systems and accessing community resources. Collaborate with medical professionals, therapists, and educators to ensure coordinated, holistic care. Advocate for children's and families' needs and educate caregivers on child development and mental health. Document assessments, services, and progress in compliance with agency requirements. Participate in staff training, community outreach, team meetings, and ongoing professional development. Licensed Clinical Social Worker (LCSW) in the state of Connecticut. Bilingual (Spanish) preferred. Experience in pediatrics, particularly working with children with autism. Knowledge of community resources that support families and children with developmental disabilities.
