Match Daily Pay Available Social Worker Responsibilities:

As a Social Worker at Boca Pointe Center for Rehabilitation and Healing , you will be responsible for assessing, counseling, interviewing, and educating patients/residents and their families. You will also assist with planning, organizing, and implementing facility policies and procedures to ensure the medically related emotional and social needs of each patient/resident are met.

Responsibilities include:

Assess patients' situations through interviews and review of personal history. Develop, coordinate, and implement individualized support plans. Develop and implement individual treatment plans. Assist patients/residents and families with Medicaid and Medicare applications. Assist with Food Stamp, housing, and telephone assistance applications as needed. Interview patients/residents and assess their current needs, strengths, and challenges. Establish individualized, measurable goals for each patient/resident. Monitor and evaluate patient/resident progress and modify plans as appropriate. Provide information, support, and counseling regarding available resources and services. Facilitate referrals to appropriate healthcare professionals and community programs. Maintain accurate and timely patient documentation. Serve as a key member of the interdisciplinary team. Promote and protect patient/resident rights, privacy, dignity, and well-being. Support a safe, secure, and respectful environment. Participate in teamwork and facility initiatives. Comply with all facility policies, procedures, and the Code of Conduct.

Qualifications:

Master of Social Work (MSW) required Experience in healthcare, long-term care, rehabilitation, or skilled nursing preferred. New graduates with an MSW are welcome to apply. Strong communication, organizational, and interpersonal skills. Ability to work effectively as part of a multidisciplinary team. Commitment to providing compassionate, patient-centered care.
