Social Worker/Care Companion Join Carease Health - FL in Dunedin, FL Are you a compassionate and dedicated Mental Health Provider looking to make a direct impact on the lives of older adults? Carease Health is seeking a caring and empathetic individual to join our Collaborative Care team as a Social Worker/Care Companion. In this role, you'll provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. About the Role As a Social Worker/Care Companion, you'll be responsible for: Visiting designated facilities in accordance with the schedule set with your supervisor Conducting assessments of residents to identify emotional, cognitive, and psychosocial needs Implementing short-term, solution-focused therapy and supportive interventions to address immediate concerns Documenting visit notes and maintaining detailed records of interactions and interventions after each visit Collaborating with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP Requirements Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings Strong communication, documentation, and relationship-building skills
