A Family, School, or General Social Worker provides specialized assistance to vulnerable families and children. Intervenes in crisis situations; evaluates individual and family needs, helps clients get access to health care, mental health care or social services such as housing assistance. Advocates for clients and provides follow up visits. May work with teachers, parents and school administrators to help students improve academically and deal with social issues such as bullying.

St. Francis Hospice (SFH) is growing, and we need a Social Worker to join our team. SFH is a well-established, mission-driven healthcare organization dedicated to providing exceptional hospice and palliative care services throughout our community. The Social Worker position is responsible for identifying the psychosocial needs, goals and interventions of patients/families. In cooperation with the Interdisciplinary Team, a range of services may be provided including, but not limited to: assessment, counseling, advocacy, education, resource and referral services, and discharge planning.
EDUCATION AND/OR TRAINING REQUIRED
Master's Degree in Social Work from a CSWE accredited school
REQUIRED.
Bachelor's education may be in a related field such as counseling, psychology, sociology, gerontology.
