A Family, School, or General Case Manager coordinates the delivery of a range of social and health services to individuals and families. Assess client needs and work with agencies and institutions ensure clients get the help they need. Monitor client and work with other service providers as necessary to address client needs. Provide limited counseling to client and their family.
Shelter Manager Position Summary The Family Coordinator provides direct support, advocacy, crisis intervention, family-centered services, and housing stability assistance to survivors and their children. This position assists with transportation, case support, housing navigation, supportive services, and daily client needs while helping ensure successful outcomes from intake through program exit. The Family Coordinator supports agency goals and compliance requirements under DCA Stable Housing Accountability Program (SHAP) funding and other grant-funded services. Essential Duties and Responsibilities Client Support & Advocacy Transport clients to appointments, work, school, court appearances, Fair Haven Market, housing appointments, and other approved activities as needed while ensuring all safety protocols and child restraints are properly used. Encourage, empower, and support clients during interactions and transportation services. Accompany clients to appointments, court proceedings, housing appointments, and community services as needed and advocate on their behalf. Assist clients with applications, obtaining identification and necessary documentation, preparing resumes, job searches, and connecting with community resources. Assist clients and children with identified needs and provide ongoing support throughout services. Housing Stability & DCA SHAP Responsibilities Assist participants in locating and securing safe, stable, and affordable housing. Support clients with housing applications, lease paperwork, landlord communication, utility applications, and obtaining documentation required for housing placement. Assist with housing barrier assessments and identify strengths and needs related to maintaining long-term housing stability. Coordinate supportive services that may include transportation, employment assistance, budgeting support, childcare resources, and community referrals. Assist with client move-ins and move-outs, including coordinating furnishings, household items, and housing setup needs. Monitor participant progress toward housing goals and provide ongoing support to reduce barriers to housing retention. Conduct follow-up contacts with participants after housing placement when required by program guidelines. Work collaboratively with landlords, community partners, and service providers to support housing stability and successful outcomes. Assist with tracking and maintaining documentation related to housing assistance and supportive services provided through SHAP and other funding sources. Shelter and Family Services Assist with medication distribution according to agency policy and accurately document medication administration and distribution times. Help ensure clients and children have access to clothing, medical care, educational resources, and other necessary supports. Provide immediate crisis intervention and assist with client intake and assessment processes. Orient new shelter clients to the shelter layout, expectations, policies, and daily routines. Provide age-appropriate comfort items and emotional support to children entering services when available. Assist Case Managers with implementation of client case plans and support successful outcomes from intake through discharge. Assist with support groups, parenting groups, and family-centered programming within shelter and outreach settings. Assist with holiday activities and special events for families and children. Documentation & Compliance Complete accurate, timely, and thorough documentation in CaseWorthy, ClientTrack and any additional required reporting systems daily. Maintain complete client files and ensure documentation meets agency, state, federal, and grant compliance standards. Answer and assess crisis hotline calls and assist with safety planning, shelter screening, referrals, and outreach services. Assist with collecting required supporting documentation for housing and supportive service programs. Maintain organized records and ensure documentation accuracy for audits, monitoring visits, and reporting requirements. Participate in case staffing and assist with outcome tracking and reporting. Additional Responsibilities Attend all required trainings including crisis-line training, trauma-informed care training, DCA-required trainings, and continuing education opportunities. Assist with lifting and moving donations, household items, furniture, and supplies as needed. Demonstrate commitment to the mission and goals of the organization. Maintain strict confidentiality of all clients, staff, and agency information during and after employment. Submit to criminal background checks and all employment screening requirements. Maintain a valid driver's license, reliable transportation, and required automobile liability insurance. Some evening, weekend, after-hours work, travel, and on-call responsibilities may be required. Perform other duties as assigned by immediate supervisor or Executive Director. Preferred Qualifications Associate degree or higher in Social Work, Human Services, Psychology, Criminal Justice, or related field preferred. Experience in victim advocacy, crisis intervention, housing programs, case management, or related services preferred. Knowledge of trauma-informed care and housing stability practices preferred.
