Job Description

Role Overview:
The social worker provides assessment, care planning, and case coordination services to patients and families, serving as a liaison between healthcare providers and the community. Responsibilities include developing and evaluating social work plans, offering psychosocial assessments, diagnosing, and treating individuals, couples, families, and groups to promote social and emotional well-being. The role involves case management, resource referral, advocacy, and coordination of services to ensure appropriate patient placement and support. Additional duties include program development, policy implementation, quality improvement, research, documentation, and participating in departmental activities. The position requires collaboration with staff and ongoing professional development.
Qualifications & Skills:
Master's degree in social work Licensed Clinical Social Worker or Licensed Social Worker (Hawaii) Basic Life Support certification from the
American Heart Association Preferred:
minimum one year of social work experience, healthcare background with medical diagnoses, and strong community agency networking skills
