ESSENTIAL JOB FUNCTIONS

Performs comprehensive, in-depth assessment and clinical formulation, development of accurate diagnosis and strengths-based treatment plans for CCBHC services. Performs open access as assigned. Provides individual/group therapy, crisis intervention, and case management services, adhering to CCBHC and MRO/Medicaid rehabilitation service models. Provides a full range of therapeutic interventions, advocacy, skill-building, and related services. Provides evidence-based treatment interventions per training and certifications. Learns and utilizes agency and community resources. Assesses community conditions that affect clients to identify resource gaps and problems. Works with multidisciplinary professionals including psychiatrists, nurses and community agencies to provide a continuum of coordinated care to meet the complex needs of clients. Recommends financial assistance and other benefits the client may be entitled to. Maintains timely and accurate client records and any necessary agency and CCBHC required reporting. Works effectively with diverse staff and service population.

QUALIFICATIONS

Enrolled in a master's degree, Social Work, Counseling from an accredited college or university required. Vehicle required. Ability to work effectively with diverse individuals and groups. Must work evenings and weekends as assigned.

PHYSICAL DEMANDS

While performing the essential duties of this job, the employee is required to sit and stand for sustained periods of time, frequently talk, hear, use hands and fingers to feel, handle and operate objects or controls, and occasionally stand, bend, stretch, stoop, kneel and crouch. The employee is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing and viewing a computer terminal.
