Full Job Description We Are Hiring:

Carmel, IN Facility:
Ascension St.
Vincent Carmel Department/Specialty:
Case Management Social Work; Units covered: Mother baby, NICU, ED and floor consults
Schedule:
Days | PRN | Monday - Friday 8:00am - 4:00pm
What You Will Do:
Architect personalized care journeys by integrating patient values and clinical needs into a seamless transition plan from admission through discharge. Empower vulnerable populations by providing expert intervention and resources for complex needs, including crisis support, substance abuse recovery, and legal advocacy. Eliminate barriers to recovery by proactively advocating for community resources and insurance coverage to ensure every patient has what they need to thrive at home. Lead the interdisciplinary care team through continuous dialogue and collaboration, ensuring the health plan evolves alongside the patient's progress. Champion clinical accuracy by serving as the expert resource for medical documentation and post-discharge service levels, ensuring care is both safe and appropriate.
What You Will Need:
Licensure / Certification /
Registration:
One or more of the following required: Clinical Counselor obtained prior to hire date or job transfer date. Counselor obtained prior to hire date or job transfer date. Social Worker credentialed from the Indiana Behavioral Health and Human Services Licensing Board obtained prior to hire date or job transfer date. Social Worker specializing in Clinical Practice credentialed from the Indiana Behavioral Health and Human Services Licensing Board obtained within 12 Months (1 year) of hire date or job transfer date. Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. Case Manager credentialed from the American Case Management Association (ACMA) preferred.
Education:
Bachelor's degree in Social Work required.
Work Experience:
3 years of experience preferred.
Additional Preferences:
Medical Social Work experience highly preferred.
