Neighborhood Intake Coordinator Geminus Corporation partners with community-based health care providers, social service organizations and early childhood education programs to help increase access to high-quality social health care for children and families across Northern Indiana. Geminus Corporation is a member of the Regional Care Group. Position Purpose Responsible for the supervision of Community Partners for Child Safety, and may provide support for new initiatives. Oversee community, DCS referrals and cases, Juvenile Justice Referrals and cases staffing, child and family team meetings and staff supervisor. The area covered will include but is not limited to the following counties, St. Joseph, Elkhart, and Benton counties 1. Provide early intervention services to expectant parents and infant caregivers. 2. Provide documentation of services rendered, upon request to the ISDH (Indiana State Department of Health), Department of Child Services and other stakeholders. 3. Submit quarterly reports and monthly documentation of Pak N' Plays/Survival Kits distributed to each family including complete safe sleep questionnaires. 4. Increase awareness regarding health behaviors that impact Lake County Indiana's Infant Mortality Rate. 5. Participate in any conference calls, webinars, and onsite meetings scheduled by the ISDH (Indiana State Department of Health) Safe Sleep staff. 6. Provide documentation that each Safe Sleep trainer has been appropriately trained in the standard education provided to caregivers. 7. Provide education, counseling, follow up services to families, expecting parents and newborn caregivers. 8. Promotion of the Indiana Safe Sleep program/outreach and marketing. 9. Provide home-based, one-on-one, and classroom setting Infant Safe Sleep classes, with the focus on the following: 10. Hands on demonstration for Pak N' Play assembly 11. Education on SIDS (Sudden Infant Death Syndrome), SUID (Sudden Unexpected Infant Death), and Purple Period of Crying 12.Safe Sleep Video (Rachel Moon, M.D.) 13.

Pre/Post Safe Sleep Assessments Minimum Hiring Requirements Education :

Bachelor's degree in social work, psychology or related social service degree.

Experience:

Minimum of 3 years experience working within the child welfare system and community agencies. Pay Range $21.04/hour
