The ideal candidate will have a strong background in senior living, experience working with an interdisciplinary team, and discharge planning. Responsibilities
  • Conduct assessments to determine resident needs, preferences and and develop care plans
  • Collaborate with the interdisciplinary team to ensure comprehensive care for residents
  • Advocate for resident's rights and lead in-service and orientation efforts related to resident rights, abuse, neglect, exploitation and trauma informed care
  • Assist residents in accessing resources such as DME, therapy services, and other social services
  • Implement psychosocial interventions proactively
  • Initiate and lead care plan meetings Skills and Qualifications
  • Bachelor's or Master's degree in Social Work or related field
  • State licensure as a Social Worker
  • Proven experience in senior living
  • Strong knowledge of Medicare, discharge planning, and other social service programs
  • Excellent communication and interpersonal skills
