The ideal candidate will have a strong background in senior living, experience working with an interdisciplinary team, and discharge planning. Responsibilities

Conduct assessments to determine resident needs, preferences and and develop care plans
Collaborate with the interdisciplinary team to ensure comprehensive care for residents
Advocate for resident's rights and lead in-service and orientation efforts related to resident rights, abuse, neglect, exploitation and trauma informed care
Assist residents in accessing resources such as DME, therapy services, and other social services
Implement psychosocial interventions proactively
Initiate and lead care plan meetings Skills and Qualifications
Bachelor's or Master's degree in Social Work or related field
State licensure as a Social Worker
Proven experience in senior living
Strong knowledge of Medicare, discharge planning, and other social service programs
Excellent communication and interpersonal skills
