Acute Care / Hospital Job Summary We are seeking a compassionate and dedicated Care Manager (RN) or Social Worker (BSW) to join a collaborative acute care team. In this role, you will coordinate patient care, assess healthcare needs, develop individualized care plans, and ensure patients receive appropriate, timely, and high-quality services throughout their hospital stay. The ideal candidate is committed to delivering patient-centered care, collaborating with interdisciplinary teams, and helping patients and their families navigate the healthcare system while promoting positive health outcomes. Key Responsibilities Coordinate and manage individualized patient care plans. Assess patient needs and determine the appropriate level of care. Determine the appropriate level of care based on patient needs. Collaborate with physicians, nurses, and interdisciplinary teams to ensure coordinated care. Monitor patient progress and evaluate the effectiveness of care plans. Assist patients and families with discharge planning and community resource coordination. Promote quality, efficiency, patient safety, and positive health outcomes. Maintain accurate documentation and comply with regulatory and organizational standards.
