Job Description

Job Overview We are seeking a dedicated and compassionate Certified Social Worker to join our dynamic healthcare team. In this vital role, you will provide comprehensive social work services to diverse patient populations, supporting their emotional well-being, facilitating access to community resources, and ensuring high-quality patient care. Your expertise will help improve patient outcomes, promote recovery, and enhance overall quality of life across various clinical settings. Responsibilities Conduct thorough patient assessments to identify social, emotional, and environmental needs, ensuring adherence to social work practice standards and HIPAA regulations. Develop personalized care plans that incorporate behavioral therapy techniques, motivational interviewing, and crisis intervention strategies. Coordinate with healthcare teams to facilitate discharge planning, community resource linkage, and post-acute care arrangements. Provide individual and group counseling sessions utilizing clinical counseling methods such as cognitive behavioral therapy and applied behavior analysis. Document all medical and social work interactions accurately within electronic medical records (EMR) systems while maintaining confidentiality in accordance with HIPAA guidelines. Engage in case management activities including intake procedures, diagnostic evaluations, and ongoing client communication to support mental health and behavioral health goals. Collaborate with multidisciplinary teams on complex cases involving hospice & palliative medicine, geriatrics, pediatrics, or individuals with disabilities such as autism or developmental delays. Requirements Valid certification as a Licensed Clinical Social Worker (LCSW) or equivalent credential. Proven experience working with diverse populations including geriatric patients, children, or individuals with developmental disabilities. Strong knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and Medicare regulations. Familiarity with clinical research practices, data collection methods, and documentation standards in healthcare settings. Experience in hospice care, end-of-life support, crisis management, and home & community care environments preferred. Ability to perform patient assessments using diagnostic evaluation tools and deliver effective psychotherapy or behavioral health interventions. Excellent communication skills for client engagement and interprofessional collaboration within inpatient or outpatient settings. Pay:
$47,000.00 - $74,857.84 per year
