Key Responsibilities Provide social work services to patients and residents in accordance with their individual needs and care plans. Conduct psychosocial assessments and identify social, emotional, and environmental needs. Develop and implement individualized social work interventions and care plans. Provide emotional support, counseling, and crisis intervention to patients and families as appropriate. Assist patients and families with adjustment to illness, disability, changes in functional status, and long-term care. Serve as an advocate for patients' rights, preferences, dignity, and quality of life. Coordinate discharge planning and assist patients and families with transitions of care. Connect patients and families with appropriate community resources, benefits, services, and support programs. Collaborate with nursing, rehabilitation, medical, and other interdisciplinary team members. Participate in care planning meetings and interdisciplinary team conferences. Address patient and family concerns promptly and professionally. Maintain accurate and timely documentation of assessments, interventions, progress, and care coordination. Assist with advance directives, guardianship matters, and other psychosocial needs as appropriate. Participate in quality improvement initiatives and facility programs. Follow all applicable federal, state, and facility regulations, policies, and procedures. Maintain confidentiality and protect patient information in accordance with applicable privacy requirements. Job Requirements Current and valid Massachusetts Licensed Social Worker (LSW) license . Bachelor's or Master's degree in Social Work from an accredited program, as required by applicable regulations. Previous experience in skilled nursing, long-term care, rehabilitation, healthcare, or a related setting preferred. Strong knowledge of social work principles, patient advocacy, care coordination, and community resources. Excellent communication and interpersonal skills. Compassionate and patient-centered approach. Strong organizational and time-management abilities. Excellent documentation and record-keeping skills. Strong problem-solving and critical-thinking skills. Ability to work effectively with patients, families, caregivers, and interdisciplinary healthcare teams.
