Provide case management to short-term and long-term patients. Assist with admissions and discharge planning processes. Plan and attend care conference meetings with patients and their support systems. Maintain effective communication with a multidisciplinary team, patients, and their support systems. Help with completing advance directive documents, if needed/desired. Document interactions and efforts made on behalf of patients. Track physician certifications of capacity and MOLST documents to ensure compliance with state regulations.
