Social Worker LCSW-C Job Summary:
Lions Rehab Center, a beautiful 101-bed skilled nursing facility in Cumberland, Maryland, is seeking a compassionate and dedicated Social Worker to join our team. As a Social Worker, you will play a vital role in providing social work services to our residents and patients, focusing on their physical, emotional, and social well-being. Responsibilities:
Plan, develop, organize, implement, evaluate, and direct social services programs of the facility Develop, administer, and coordinate department policies and procedures Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Administrator Participate in community planning related to interests of the facility and services and needs of residents and family Perform administrative requirements, such as completing necessary forms, reports, etc. and submitting them as required Make written and oral reports/recommendations to the Administrator and relevant department heads concerning the operation of the social service department Be prepared to handle emergencies as they arise (i.e. rescheduling work assignments and work) Orient residents and family to the Nursing Center, helping both adjust to the new environment Develop a social history on each resident Develop social assessments and care plans; which identifies medically related social and emotional problems and needs with realistic goals and specific actions to be taken Answer inquiries of admission and provide tours to prospective family members/residents Be actively involved in pre-admission and admissions processes Assist in continually updating the admissions waiting list Interview residents and family members as necessary and in private settings Ability to listen and act compassionately with residents' and family members; must be able to work on own and maintain related jobs Why work for us? Requirements:
