Social Worker-LMSW Palliative Care - Position Summary The Palliative Care Social Worker provides psychosocial support to Palliative Care patients and their families, including patients admitted to the hospital. The Social Worker assists the with coordination of outpatient palliative services and hospice referrals, and makes outpatient referrals as needed, ie dietitian, social work, other. The Social Worker assists in the triage of new consult patients based on reason for consult. This position documents patient SW assessments, family meetings, interactions, family dynamics, and other information in the medical record. The Social Worker assists patients with completing advanced directives and/or aids in obtaining previously completed advanced directives. Social Worker-LMSW Palliative Care - Position Requirements Education Master's degree in Social, psychology, sociology or related field is required. LMSW required.
LCSW-C/LCPC
licensure preferred. Experience Experience working within Palliative care is preferred Social Worker-LMSW Palliative Care - Schedule Primarily day shift, Monday through Friday. May rotate weekend hours and work overtime if needs of patients situation require. Social Worker-LMSW Palliative Care - Benefits At TidalHealth, team members working at least 36 hours per pay period based on 12-hour shift schedules or at least 37.5 hours for non 12-hour shift schedules and part-time team members working at least 30 hours or more on weekends only are eligible for benefits.
