In this vital role, you will provide comprehensive mental health assessments, evidence-based interventions, and case management services to diverse patient populations. Your expertise will support individuals with mental health challenges, developmental disabilities, substance use disorders, and co-occurring conditions, ensuring they receive holistic care that promotes recovery and well-being. This position offers an exciting opportunity to make a meaningful impact through patient-centered care, clinical research, and community engagement. Professional visit shall pay $ 30-35 and last a minimum of thirty (30) minutes to be compensated . Responsibilities Conduct thorough mental health and behavioral assessments utilizing diagnostic tools such as ICD-10 and ICD-9 coding systems, ensuring accurate documentation in EMR/EHR systems while maintaining strict adherence to HIPAA and clinical confidentiality policies. Develop and implement individualized treatment plans incorporating evidence-based practices like cognitive behavioral therapy, psychodynamic therapy, applied behavior analysis, and motivational interviewing tailored to each patient's unique needs. Provide direct counseling services including individual therapy, group therapy, child & family counseling, and specialized interventions for autism spectrum disorder, Alzheimer's care, PTSD, and substance use disorders. Coordinate care through case management activities such as intake assessments, patient education, crisis intervention, and community resource linkage to support patients' social determinants of health. Collaborate with multidisciplinary teams—including medical professionals, educators in special education settings, and community organizations—to ensure comprehensive patient care across home and community environments. Collect data for clinical research initiatives focusing on behavioral health outcomes, addiction counseling effectiveness, or developmental disabilities to inform continuous quality improvement efforts. Maintain detailed clinical documentation following medical records standards while ensuring compliance with medical terminology conventions and coding practices like CPT coding for billing purposes. Qualifications Current Michigan license as a Limited or Fully Licensed Social Worker by the Michigan Department of Licensing and Regulatory Affairs. At least one year of experience providing Licensed Social Work services to families. Experience in the field visiting clients and case management preferred. Familiarity with EMR/EHR systems for documentation and data collection; ability to adapt quickly to new technology platforms is essential. Excellent communication skills for engaging patients across age groups—from pediatrics to geriatrics—and collaborating effectively with multidisciplinary teams. Ability to perform comprehensive mental health assessments using ICD coding standards (ICD-10/ICD-9), ensuring accurate clinical documentation aligned with healthcare regulations.
