Job Description

New Paths, Inc. is looking to fill a full-time, non-exempt position for an SUD Home Health worker. See description below.
Job Summary:
The SUD Home Health Coordinator works with clients as an advocate and acts as a liaison in providing appropriate community resources. The SUD HH will identify and gather community resources, observe/participate in client assessments and action plan.
Duties/Responsibilities:
Support and adhere to the NPI Code of Ethics, work rules, policies, plans and procedures to promote a safe and secure environment for the residents, visitors and employees of New Paths. Identify and gather appropriate community resources, including but not limited to mental health, SUD, medical (health education), housing, financial resources. Provide problem-solving techniques with clients. Meet with and communicate with clients regularly. Participate in health promotion activities or health education class Observe/participate in client assessments. Assist in implementing client action plans. Develop and maintain education materials as resources for staff and clients. Develop and maintain cooperative working relationships with agencies and organizations in the community. Document activities and record information. Other duties as assigned.
Required Skills/Abilities:
Excellent interpersonal and customer service skills. Ability to interact professionally and respectfully with staff and clients. Excellent verbal and written communication skills. Proficient in Microsoft Office Suite or related software. Ability to work independently. Ability to coordinate and effect a smooth flow of behavioral-based information and data Skill in communicating with a variety of people from varying backgrounds Excellent organizational skills, including the ability to deliver information and services in a timely fashion. Time management. Exhibit problem solving and conflict resolution techniques.
Education and Experience:
Minimum of 3 years' experience in health services field required, SUD experience preferred. CCAR or MDHHS peer recovery coach certification required. CHW Certification, preferred.
High School Diploma or GED Physical Requirements:
Prolonged periods sitting at a desk and working on a computer. Periods of walking throughout building. Ability to climb stairs as necessary. Must be able to lift up to 15 pounds at times.
