Job Duties:

Assess and address residents' psychosocial needs Develop and implement individualized care plans Provide emotional support and advocate for resident rights Coordinate discharge planning and community referrals Maintain accurate documentation and attend care conferences Collaborate with interdisciplinary teams and families Ensure compliance with all regulatory standards

Qualifications:

Bachelor's degree in Social Work, Sociology, or related field Licensed Social Worker in the state of employment Minimum 2 years of healthcare experience (long-term care preferred) Strong communication, organizational, and interpersonal skills Ability to manage sensitive situations with professionalism Make a meaningful impact—apply today and help us support residents with dignity and care!
