Job Description

We are currently seeking a compassionate and experienced Hospice Medical Social Worker to join our interdisciplinary team and provide meaningful, holistic support to patients and families navigating one of life's most profound journeys. The Role As a TMT Hospice Medical Social Worker, you will serve as an essential member of the Hospice Interdisciplinary Group (IDG) , delivering a full spectrum of social work services that address the emotional, spiritual, and psychosocial needs of patients and their families. Your work will extend beyond clinical care — it will offer comfort, clarity, and connection during the moments that matter most.
Core responsibilities include:
Conducting comprehensive initial psychosocial, emotional, and spiritual assessments Providing ongoing individual and family counseling throughout the continuum of hospice care Delivering sensitive and supportive bereavement services to families following the loss of a loved one Facilitating community education, outreach, and resource referrals to connect patients and families with the support they need Collaborating closely with the interdisciplinary care team to ensure a coordinated, patient-centered approach to end-of-life care Responding with professionalism and composure to crisis situations in community and home settings Qualifications Education & Licensure Master of Social Work (MSW) degree from a CSWE-accredited school of social work (required) Minimum of one year of experience in a healthcare setting (required) 2 years of recent patient contact (required) 2 years of social work experience (preferred) Knowledge, Skills & Abilities Strong capacity to work independently and exercise sound, timely clinical judgment Demonstrated ability to assess and respond effectively to crisis situations outside of a hospital environment Cultural humility and adaptability across diverse social, racial, cultural, and religious backgrounds Excellent interpersonal and communication skills with a genuine commitment to patient dignity and family support Compensation & Benefits TMT recognizes that exceptional care begins with a supported and valued workforce.
