Job Summary We are seeking a compassionate and dedicated Licensed Social Worker to join our dynamic healthcare team. In this vital role, you will provide comprehensive social work services to patients and their families, ensuring they receive the support, resources, and counseling necessary for optimal well-being. Your expertise will help navigate complex emotional, social, and medical challenges. Responsibilities Conduct thorough patient assessments to identify social, emotional, and environmental needs, utilizing clinical evaluation techniques and diagnostic tools. Develop individualized care plans aligned with clinical ethical guidelines and social work practice standards, coordinating with interdisciplinary teams to ensure seamless patient care. Provide specialized counseling services including child & family counseling, behavioral therapy, addiction counseling, and mental health support tailored to each patient's unique circumstances. Facilitate crisis intervention strategies promptly to address urgent mental health or social issues while maintaining compliance with HIPAA regulations and medical documentation standards. Coordinate community resources such as housing assistance, financial aid programs, hospice & palliative medicine services, and disability support to enhance patient outcomes. Manage case documentation accurately within electronic medical record (EMR) systems, ensuring compliance with medical terminology standards like ICD-10 coding for diagnoses. Engage in discharge planning and post-acute care coordination to ensure smooth transitions from hospital or inpatient settings to home or assisted living environments. Qualifications Valid licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Masters Social Worker (LMSW) or equivalent credential in accordance with state regulations. Proven experience working with diverse populations including pediatric patients, individuals with developmental disabilities, autism spectrum disorder, anxiety, ADHD, ODD, etc. Strong knowledge of social work practice standards, medical records management, CPT coding, Medicare policies, and HIPAA compliance. Demonstrated expertise in clinical counseling modalities such as cognitive behavioral therapy (CBT), motivational interviewing, group therapy, and crisis management techniques. Familiarity with outpatient clinical experience, inpatient or outpatient settings, home & community care environments Excellent communication skills for client interaction and clinical interprofessional collaboration Ability to handle sensitive situations with empathy while maintaining professional boundaries; proficiency in EMR systems Join us in delivering compassionate social work that transforms lives!
