Provide specialized counseling services including child & family counseling, behavioral therapy, addiction counseling, and mental health support tailored to each patient's unique circumstances. Facilitate crisis intervention strategies promptly to address urgent mental health or social issues while maintaining compliance with HIPAA regulations and medical documentation standards. Coordinate community resources such as housing assistance, financial aid programs, hospice & palliative medicine services, and disability support to enhance patient outcomes. Manage case documentation accurately within electronic medical record (EMR) systems, ensuring compliance with medical terminology standards like ICD-10 coding for diagnoses. Engage in discharge planning and post-acute care coordination to ensure smooth transitions from hospital or inpatient settings to home or assisted living environments. Qualifications Valid licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Masters Social Worker (LMSW) or equivalent credential in accordance with state regulations. Proven experience working with diverse populations including pediatric patients, individuals with developmental disabilities, autism spectrum disorder, anxiety, ADHD, ODD, etc. Strong knowledge of social work practice standards, medical records management, CPT coding, Medicare policies, and HIPAA compliance. Demonstrated expertise in clinical counseling modalities such as cognitive behavioral therapy (CBT), motivational interviewing, group therapy, and crisis management techniques.
